Patient Portal Information and Guide:
Please consider these guidelines and recommendations while using our complimentary Patient Portal provided to you as a free and secure service by Eclinical Works, our Electronic Health Records Vendor.
Family Care Associates is excited to offer a secure site online that our patients may use to access their personal health information including test results, appointment information, and statements.

Additionally, the Patient Portal allows patients to send instant messages to the practice for certain situations, eliminating phone calls, hold times or messages left on the voicemails.  If you are in need of a refill or are requesting lab results, simply log in and send us your request virtually.   You will receive an e-mail when a response has been posted.

Things to Remember:

Do not rely on messages sent through the Patient Portal during an emergency or if you need urgent care.  If you are ill or injured, please call the office to schedule an appointment.

Messages sent to Family Care Associates will be answered during regular business hours.

Your account will be automatically locked after 10 failed log in attempts as per the security features of the Portal.  You may contact the office to have your account reset, or wait until the following morning and try again.

Keep your log in information private and only share it with individuals whom you authorize to access your information.  You can reset your password at any time if necessary.  Family Care Associates will NOT know your password.

How to Access Your Portal:

You will receive a copy of our Patient Portal Policy.  You will sign a consent form to participate with the Portal and provide us with a clear, legible, NON-WORK e-mail address.  Our staff will enter your e-mail address into our electronic health records and enable your account.  You will then receive a "welcome" e-mail with log in instructions and your temporary username and password.   When you receive your login information, visit our website and click the Patient Portal link below.  You will then be routed to the Portal.

When you log in for the first time, you will be asked to agree to the "Terms of Use".  Once you agree, you will be routed to your personal Portal home page.  From there, use the menu items on the left to access messages, health information, lab results and more!

What is the Patient Portal?

The Patient Portal is a secure site online that our patients may use to access portions of their personal Electronic Health Records.  This is free, secure and available to all of our patients.  You must provide us with a legible, valid personal e-mail address and sign a Patient Portal Policy Agreement to initiate this process.
