Information for Our New Patients and Established Patients:
In 2006, Harrison Memorial Hospital celebrated 100 years of caring for the counties they serve. Once a community hospital, they have expanded their facility, acquired cutting-edge technology and partnered with physician specialists leading in their field to deliver the highest quality healthcare closer to home.

Their regional medical center now boasts a new emergency department that has doubled in size, state-of-the-art labor, delivery and recovery suites, a new newborn nursery, new chemotherapy and infusion therapy suites, new space dedicated to specialty clinics and a team of health professionals with experience unmatched in the area.

Harrison Memorial Hospital currently has 20 physicians on their active medical staff and more than 50 consulting specialists. Harrison Memorial Hospital is a locally owned and controlled, not-for-profit facility.

They are licensed by the state of Kentucky to operate 61 beds, and certified by Medicare/Medicaid. They are a member in good standing with the following agencies, and meet or exceed their standards for quality:

Please make sure to bring current insurance card, photo identification, all medications and any co-payment, deductible payment or payment in full to every office visit.

During your first visit to our office, registration information will be collected.  This information and any other information given to our office will be kept in the strictest confidence.  Please see our Privacy Policy.


Appointments

Our Cynthiana office is open weekdays from 8am to 4:30pm, until 6pm on Monday, and 8am-11:30am one Saturday per month.  Appointments are required for all care.  You can arrange an appointment by calling our office at 859-234-6000.

J. Gregory Cooper, MD is in the Cynthiana office all day Monday and Wednesday.   

Brian T. Mulberry, MD is in the Cynthiana office all day Monday and the mornings on Tuesday, Thursday and Friday. 

R. Henry Norfleet, MD is in the Cynthiana office all day Tuesday and Friday, as well as, Monday and Thursday afternoons.  

Kathy Hammond, APRN is in the Cynthiana office Monday morning, Tuesday afternoon and all day Wednesday.  She visits our nursing home residents Tuesday mornings. 

Crista Crowdy, PA-C is in the Cynthiana office Wednesday through Friday and every other Saturday.

Dr. Mulberry will be in clinic one Saturday per month in the Cynthiana office.

Appointments are not needed for flu shots, allergy injections, labwork and blood pressure checks by the nurse.
For your convenience, we recommend that you call in advance to schedule physical examinations and routine check-ups.  Other appointments are scheduled as available and necessary.  Some appointments are set aside each day for sudden illnesses, these fill quickly each morning.  Please call early.  If you are unable to keep a scheduled appointment, we ask that you notify our office as much in advance as possible.  Early notification allows someone else to use that appointment time and you to reschedule as soon as possible.  Failure to notify our office that you cannot keep your appointment time on multiple occasions, may results in a dismissal from the practice.

If you arrive more than 15 minutes late for an appointment, you may be asked to reschedule or wait until other scheduled patients have been seen.  This avoids long waits for you and other patients, and provides adequate time for a thorough visit.

If you or your family need emergency care or  urgent information outside of office hours, contact Harrison Memorial Hospital at 859-234-2300 and leave a message with the operator.  The physician on call will return your call.​  Please do NOT page the physician on call for medication refills.






Payment

Co-payments or payments in full at the time of service is expected.  If your Health Benefit Plan has a high deductible, we expect a payment of $40 to be applied toward your deductible at the time of service until the deductible is met.  You may pay by check, credit/debit card or cash.  Please make checks payable to Family Care Associates or FCA.  You will be offered a receipt for each payment.  Labwork and x-rays performed at Harrison Memorial Hospital, Combined Physicians Lab or other facilities will be billed to you separately.  Any overpayment can be credited to your account or refunded to you.

Please notify our office staff to any change in insurance when you arrive for your appointment.  If you are on Medicaid, please present your card when you arrive for each appointment.    

If you do not present your insurance card when you arrive for your appointment or are not prepared to pay your co-payment, co-insurance, or the full amount of your office visit, we reserve the right to reschedule your appointment until such a time that you are prepared to do so.

Should you be unable to make payment at the time of service, a statement will be mailed to you.  We expect prompt payment.  Patients whose accounts are 30 days past due will be contacted so that payment arrangements can be made.  We realize that special situations arise that require alternative payment plans.  Please discuss these with our Billing Coordinator before your account is overdue.





Telephone Calls

When you call our office, our office staff will direct your call as needed.  If you are requesting to speak to a nurse, you may be routed to the nurse voicemail and prompted to leave a message.  If you are requesting a refill, you may be routed to the refill  voicemail.  Our message nurse will check all nurse voicemails throughout the day.  For Referrals, Medical Record requests and Billing/Insurance issues, your call may be routed to the voicemail of the appropriate staff member.   ALL voicemails are checked throughout the day.  

Calls regarding laboratory tests or x-ray results, when performed at another facility, will be returned when the results are available.  Any abnormal results will be reported to you as soon as the physician has reviewed them.  A letter will be sent to you if all test results are normal.  These results can be reviewed at your next appointment.

Refilling Medications/Samples:  Family Care Associates requires a 48 hour notice on refilling any medications or samples.  Call your pharmacy when you are low on medication to see if you have any refills remaining.  Your pharmacist will send a request to our office if there are no refills remaining.  Please contact our office for refills on prescriptions that must be written and allow 48 hours for the refill request to be process.  

When leaving voicemails on any line, please speak clearly and say the name of the caller, the name of the patient, the date of birth of the patient.  Then  leave a valid call-back number and a detailed message.

If you call outside of regular office hours, you will reach a recorded message.  You may leave a non-emergent message for us that will be addressed the following business day.  In case of Emergency, DO NOT leave a message.  Hang up and dial 911 immediately.

Narcotics

Family Care Associates is NOT a Pain Management Facility. KY state laws require strict monitoring for controlled substance medications.  We are required to obtain KASPER reports (controlled substance history) every 3 months on all patients taking controlled substances.  We are also required to perform Random Drug Testing.  All patients prescribed controlled substance will be required to sign and abide by Family Care Associate's Controlled Substance Agreement.  Lost or stolen controlled substances WILL NOT be replaced.  Early refills of controlled substances WILL NOT be authorized.

These policies are guidelines as to how we can work with you to achieve the best health care possible.  Cooperation and mutual understanding is vital.  If you have any questions or concerns, please feel free to ask.
